X12 272 — Property and Casualty Loss Notification
The 272 notifies a P&C loss — auto accident, home damage, business interruption — from a policyholder or agent to an insurer, for claim opening.
Purpose
The 272 documents the EDI First Notice of Loss (FNOL): nature of the loss (collision, theft, fire, flood, liability), date and location, vehicles or property involved, parties (insured, claimant, witnesses), estimated severity. It opens the claim file and triggers the claim handling cycle.
It feeds the claim management system (Guidewire ClaimCenter, Duck Creek Claims). Acknowledged by 997. Followed by 112 (Property Damage Report), 257 (Underwriting Report), and 820 (Payment).
Envelope structure
The 272 travels within the three X12 envelope levels (ISA/IEA,
GS/GE with functional group PL, and ST/SE).

Common pitfalls
- Statute of limitations: delaying a 272 exposes to denials under late reporting; trigger within 24-72 h per state rules.
- Coverage in force check: a 272 does not validate the active policy — a prior eligibility check is needed, otherwise the opened claim is denied post-facto.
- Subrogation flag: if another party is at fault, the 272 must declare subrogation potential, otherwise rights of recovery are compromised.
