DPR^O48 — Donor Referral

Referral of a donor (blood, plasma, organ, hematopoietic stem cells) from a referring center to a receiving center. Core activity of blood services (EFS in France) and marrow donor registries (France Greffe de Moelle / Match-IS / NMDP US).

Purpose

DPR^O48 transmits the complete donor record (serology status, ABO/Rh, HLA, medical history) between two organizations: regional EFS to national EFS, or collection center to transplant center. Enables cross-center matching optimization and regulatory traceability (EU Directive 2002/98/EC, FDA 21 CFR 1271).

Segment structure

Structure DPR_O48: MSH, [SFT], [NTE], PID, [PD1], { DON, [ORC, [OBR, {OBX}]] } (1..* groups).

  • DON-1 — Set ID — Donation.
  • DON-2 — Donation Type (S=Standard, A=Autologous, D=Directed, R=Replacement).
  • DON-3 — Donation Phlebotomy Issue (REGULAR/EMERGENCY/REPLACEMENT).
  • DON-4 — Donation Type (VOLUNTEER/PAID/AUTOLOGOUS).
  • DON-6 — Phlebotomy Method (Whole Blood / Apheresis / Autologous).
  • DON-9 — Donor Blood Type (ABO/Rh).
  • DON-11 — Donor Eligibility Status.

Real-world example

EFS Bordeaux refers Marie DUPONT (HSC donor) to national EFS for HLA matching:

Common errors

  • DON-9 (ABO/Rh) missing: transfusion matching impossible without basic typing.
  • HLA in free text OBX-TX: for HSC, HLA typing must be CWE coded with IMGT/HLA Nomenclature convention.
  • No ISBT 128 identifier: PID-3 should carry ISBT 128 DIN (Donation Identification Number); otherwise non-interoperable.
  • Confusion DPR^O48 vs BAR^P10: BAR^P10 handles patient accounts (financial admin), DPR^O48 the donor referral (clinical).
